 Obstetrics

Postnatal Depression

  • Can occur at any time but usually within the first few weeks or months postnatally
  • Symptoms may continue for > 1yr
  • These include:-
      • Crying / weepiness
      • Low energy
      • Irritability
      • Unstable mood, panic attacks
      • Sleep disturbances, early morning wakening
      • Suicidal thoughts
      • Fear of harming self or the baby

       

Prognosis

  • Good with early diagnosis & treatment
  • > 70% recover within 1 year

 

Treament

  • Provide psychological support & practical help
  • If depression is severe consider the use of antidepressants
  • Tricyclic antidepressants are often the treatment of choice
  • Effects should be seen within the first two weeks
  • Treatment should be continued for 6 months following recovery and then gradually reduced
  • Medication can be passed through breast milk – therefore careful choice of drugs is needed
  • Hospitalisation is rarely required
